Full payment is due at the time of service. Visa, MasterCard, American Express, Check,
PayPal and cash are accepted.
For regular patients, a credit card must be on file.
I am no longer a contracted provider with any insurance company and do NOT accept
health insurance as a form of payment.
In the interest in complete patient confidentiality, I do not keep traditional medical
records that an insurance company could request for reimbursement.
I am happy to provide an invoice of services for reimbursement from health spending
accounts and/or insurance. It is recommended that you contact your insurance
company prior to scheduling an appointment to see if reimbursement is possible or for
any additional assistance.
